How to Set Up Cleats: Position Your Shoe Cleats Correctly

The cleat is the only contact point between foot and pedal — it decides where your foot sits on the pedal axle and at what angle. A cleat that's off by a few millimetres is one of the most common causes of knee pain, calf cramps, forefoot numbness ("hot foot") and Achilles tendon irritation. There are three axes to set: fore/aft, side-to-side and rotation.

1. Fore/aft: ball of the foot over the pedal axle

The starting point: the first metatarsal head (the bony bump at the ball of the foot, MTP1) sits over or slightly ahead of the pedal axle. More precisely, the pedal axle sits about 5–10 mm behind that point.

For large feet (roughly EU 44 and up) and for long distances, the rearward position tends to help. Note: moving the cleat back effectively lengthens the leg through the stroke — afterwards, re-check saddle height and drop it 2–4 mm.

2. Side-to-side: stance width and Q-factor

The lateral cleat position moves the heel inward or outward. Goal: the knee tracks straight over the foot through the stroke, without drifting in or out.

Rule of thumb: first give the heels as much clearance as needed, then correct in 1 mm steps. If you need a very wide stance, the cleat alone won't get you there — longer pedal axles or spacers do.

3. Rotation and float

Most people don't stand with perfectly parallel feet. Cleat rotation sets the foot angle to match your natural stance (heels slightly in or out). Twisted the wrong way, it forces the knee onto an unnatural path — typically causing pain on the inside or outside of the knee.

Float is the released rotational angle (0°, 4.5° or 6–9° depending on the system). Some float (4.5–6°) gives the knee room to move and forgives small setup errors. Only ride fixed 0° cleats with carefully measured rotation.

Self-check by video analysis

Set your phone up side-on and film a few crank revolutions at a normal cadence. The analysis measures your ankle and knee angle across the whole stroke and flags left–right asymmetries — a clear side difference in knee angle often points to skewed cleat rotation. Do it in the right order

  1. Set the ball position (fore/aft) the same on both shoes.
  2. Match rotation to your natural foot angle, keep float centred.
  3. Fine-tune stance width via the lateral position.
  4. Then re-check saddle height and saddle position.
  5. Ride for 1–2 weeks, change only one thing per adjustment.
